Hudson and nephews, Rick Burroughs and Paul Sturdivant. Pallbearers will be grandsons, Jason Channell, Zach Oswalt and Max Oswalt, brother-in-law, J.W. She was a woman of faith and prayer which helped her through her battle with breast cancer. She was an integral part of her grandchildren's life during their early years and continued to be an inspiration to the entire family. Hazel was a loving wife, mother, grandmother, sister, and friend to all. Hudson brother, Jim Moore grandchildren, Jason Channell (Casey), Zach Oswalt, Max Oswalt two step grandsons, Johnny and Wim Jenkins, five step great-grandchildren, Daina, Martha, Griff, Florian and Finn and numerous nieces, nephews, and extended family. Cook daughters, Sherry Jenkins (Peter) and Melissa Oswalt (Gary) very special sister and brother-in-law, Jan and J.W. She was preceded in death by her son, Phil Cook her parents, Harvey and Effie Moore and her brothers, Don, Doug and Howard Moore. Visitation will be one hour prior to services at the funeral home. Visitation will be 6-8PM Friday at Evans Funeral Home. Brent Zessin will officiate the ceremony. A funeral is scheduled for 2PM Saturday Januat Evans Funeral Home, with a reception to follow at The Clubhouse. He served as a Mason with Fidelity Lodge #685, a volunteer fireman for Florala Volunteer Fire Department and as a Deacon at First Baptist Church Florala for more than 35 years. He retired from the Air Force in 1971 while stationed in Shrevport, LA and returning to Florala, to continue to serve for another 20 years in Civil Service at Eglin Air Force Base and Hulbert Field. Shortly before the birth of daughter, they returned to Florala. After enlisting in the Air Force and completing basic training in San Antonio TX, he had tour of duties in Iceland, Iwo Jima, and Thailand, then were stationed in Bermuda where they enjoyed many years with their young son. He married Ima Jean Morgan, his high school sweetheart on June 16, 1951. He graduated from Covington County High School in 1951. Johnny Ray was born on Septemin Samson, AL. ![]() He is preceded in death by his wife of 64 years Ima Jean Cook, of Apopka, FL and his parents John W. John is survived by children Phil Cook of West Palm Beach, FL and Pam (Mark) Oldham, of Holly Springs, NC two grandchildren-Morgan Leigh and Trey Oldham two sisters-Bobbie (Buford) Milby and Karen (George) Faas. “Johnny Ray” Cook died as a patient of Florida Hospital on Decemin Altamonte Springs, FL at the age of 84.
